The following schools have not turned in their EAP Verification Form for the 2009-2010 school year. This form was due into the NCHSAA in March. These forms must be faxed into Que at 919-962-7812 by Friday, June 19th to avoid a fine. The form can be found under select a topic, sports medicine.
